A Wakefield roofing company has been fined and its sole director given a suspended prison sentence after a dad-of-two was killed when he fell 12 metres through a skylight. Jonathan May, 39, from Horbury, Wakefield, who was a subcontractor for Davis Industrial Roofing Limited, was working on a storm-damaged warehouse roof at F&G Commercials Limited, Carlton Industrial Estate in Barnsley with two others on 18 December 2016, when he fell.

On 8 January 2018, a worker at Pan Glo (UK) Ltd’s premises in Skelmersdale, Lancashire – previously known as Cleanbake Ltd – fell into a tank of water heated to 76°C, suffering 37% burns. Despite being pulled from the tank by a colleague almost immediately and then being transferred to hospital, the gentleman died from his injuries a week later. In an exclusive interview, Health and Safety Executive inspector Rose Leese-Weller explained to IOSH magazine exactly what happened.

Wood-based panels manufacturer Norbord, part of Canadian group West Fraser, has been fined £2.15m for health and safety failings leading to the death of an employee. Perth Sheriff Court found Norbord Europe Limited guilty of two charges under health and safety legislation for the accident on July 12 2016 at the company’s site in Cowie, Stirlingshire.

Two companies have been fined a combined total of more than £360,000 after lorry driver was killed when a pack of steel gates fell onto him from a trailer. Andrew Bayley-Machin, of Park Drive, Cheadle, suffered fatal chest injuries while unstrapping the load in June 2018. On Monday, the two firms were fined after admitting health and safety failures in a hearing at North Staffordshire Justice Centre.

A Blackburn building firm has been fined £116,666 after a long history of health and safety failings, including on a site in Middleton. An investigation by the Health and Safety Executive (HSE) found Mullberry Homes Limited failed to plan, manage, and monitor health and safety work on construction sites across England.

In January 2018 the Health and Safety Executive (HSE) undertook a proactive inspection to investigate health and safety failings by S&S Quality Building Contractors Limited at a construction site at Regent House, Brentwood, Essex after a concern was raised that people were sleeping on site. The subsequent HSE investigation, which was assisted by Essex Fire & Rescue Service, identified failings in fire management at the site which created risks to workers and members of the public who were visiting show flats outside of business hours.

EDDIE Stobart has released a statement following the company receiving a fine for exposing workers to asbestos at a site. The logistics company pleaded guilty to charges at the beginning of November, and admitted it did not provide relevant surveys at the time in question.

Two companies have been fined after a heavy goods vehicle (HGV) driver suffered fatal chest injuries while unstrapping a load on a trailer. Andrew Bayley-Machin, 41, of Park Drive, Cheadle, Stoke-on-Trent was fatally crushed when he was hit by a pack of steel gates that fell approximately three metres from a load on a flatbed trailer.

A transport company has been fined £400,000 after one of its drivers was killed after being knocked off his trailer while loading and unloading it. On 16 November 2020, Robert Gifkins, who worked for Arnold Laver & Company Ltd, was delivering timber to a company in Whaddon near Salisbury. He had climbed onto the bed of his trailer to sling the load and attach it to the vehicle-mounted crane. While moving the load using the crane’s remote control he was struck by the crane and fell from the vehicle to the ground. Mr Gifkins was taken to hospital and subsequently died on 17 December 2020.

Inspectors shut down a Mullberry Homes development site in Middleton. A company director was cautioned and his firm fined more than £100,000 over health and safety failings at new build housing developments which led to workers being put 'at risk'. Work building properties on one Mullberry Homes site in Middleton was stopped by the Health and Safety Executive.
